Keep a single document with your relative’s medications, allergies, chronic conditions, blood type and the phone number of their regular doctor. Share it with one trusted neighbour and with your coordinator.

In the moment

Call the local emergency number first — no coordination service replaces an ambulance. Once your relative is safe, call our care line: we alert the receiving hospital, send the medical history and stay on the case until admission is confirmed.

After the discharge

Most readmissions happen in the two weeks after a hospital stay. We book the follow-up appointment before your relative leaves the ward and check the medication list against what they were taking before.
